Our self-funded plan document provides you with the option to include short-term disability, dental benefits, freestanding prescription drugs, basic hospital, vision care, major medical benefits and high deductible health plans (HDHP). Also, you can produce administrative services agreement (ASA). You can prepare a customized plan document(s) via the Internet (ASP), on your desktop (PC), or through our Service Bureau. Each plan document may come with summary plan description (SPD).
